Abstract

The Pusri P-IB Plant is one of the factories at PT Pupuk Sriwidjaja Palembang which was built in 1993, producing 446,000 tons of ammonia per year and 570,000 tons of urea per year. In carrying out its operations, Occupational Safety and Health (K3) is the main aspect to ensure that all workers feel safe and protected in operating the factory in accordance with work and equipment safety rules. Therefore, all workers must have a proactive culture in fulfilling K3 aspects in implementing Process Safety Management (PSM), which includes important elements, namely Process Hazard Analysis (PHA). Even though the Pusri P-IB Plant has been in operation for 30 years, the preparation of the P-IB HAZOP document was prepared from the start like building a new plant and accommodated several modifications to equipment and piping units. The Re-HAZOP of the Pusri P-IB Plant Project was implemented to identify and evaluate potential hazards and operational problems of a process, system or procedure arising from the consequences of deviation from design specifications for all operational modes. The re-HAZOP of the Pusri P-IB Plant was carried out by involving internal resources, namely experienced employees with multi-disciplinary backgrounds including operations, mechanical, instrument, electrical, process engineering and engineering design which was validated by external parties at the end of the project. The Re-HAZOP document reviews 79 nodes based on 88 P&ID pages, which consist of 23 nodes at the Ammonia Plant, 33 nodes at the Urea Plant and 23 nodes at the Utilitiy Plant which produces 79 recommendations for the Ammonia Plant, 22 recommendations for the Urea Plant and 42 pieces of recommendation in Utility Plant.
